If this is your first time applying to the University of Washington, you will need to create an applicant profile. Questions 1, 2, and 3 of the applicant profile are about your legal name(s) (your name as it appears on your passport, driver’s license, birth certificate, or other government-issued ID). It’s very important to answer these questions carefully, so that any official test scores you report (TOEFL, GRE, GMAT) can be matched to your application.

Example 1: Last Name, First Name Middle Name


Report your name in the applicant profile as:

Example 2: Last Name, First Name First Name (Multiple First Names OR Space in First Name)

If you have two first names and do not consider either of them your “middle name” OR if there is a space in your first name, you can enter both first names with the space into the First Name box.
Report your name in the applicant profile as:

Example 3: Last Name, First Name Middle Initial(s)

If you would prefer to use your middle initial, or your names are too long to fit into the applicant profile name fields, you can use the first initial of each name.

If you do not have a middle name, leave the box blank.
